Time off work (Sick/Fit note/Med3)
You only need a sick note (Med3) if you have been off work for more than 7 days.
To request a sick note (Med3), please use the link below to complete an online request through Anima. Select "I have an administrative query" and then complete the "Time Off Work – Sick Note" questionnaire.
