How to register

If you live in our Practice area and would like to register with us, please complete one of our registration forms that are available from Reception, alternatively we are able to register you via a digital form. Please complete the Online New Patient Request here.

We only use your information to set you up as a new patient and delete your submission after you have been processed.

You will be offered an appointment for a well person check with the Nursing team. We use this opportunity to collect information on your past medical history, health problems, regular medication and a base line of current health. It is also a good time to check all immunisations, cervical smears and contraceptive requirements are up to date. Unfortunately it can take up to two months to obtain your medical notes from your previous doctor so it is important we collect this information from you directly. 

Temporary Residents

We will be happy to see friends and relatives staying with you if they need to see a Doctor or Nurse.

However, we advise that the relative must contact their own home surgery for advice before contacting Whiteparish Surgery to request an appointment.
