General Practice Data for Planning and Research

To provide more time to speak with patients, doctors, health charities and others, the collection of GP data for Planning and Research in England has been deferred from 1 July to 1 September 2021

A type 1 opt out prevents information being shared outside a GP practice for purposes other than direct care. (Form avaialble below to be returned to your GP Surgery)
A type 2 opt out prevented information being shared outside NHS Digital for purposes beyond the individual's direct care.
