Privacy Notice - Medication Collection from Local Village Shops

Whiteparish Surgery is the data controller for your personal information and responsible for ensuring your information is kept secure and processed in accordance with data protection laws.

Some patients choose to collect their prescribed medication from a local village shop rather than directly from the surgery.

What information we use

To provide this service, we need to use:

  • Your name
  • Your address
  • Your date of birth
  • The village shop you have chosen as your collection point

Why we use your information

We use your information to:

  • Prepare and package your medication
  • Deliver it to your chosen collection point
  • Ensure that the correct medication is provided to the correct patient

This service is provided only at your request.

The lawful basis for processing your information is that it is necessary for the provision of health care and the management of health services. By requesting collection from a village shop, you are asking us to share the minimum information necessary to facilitate this service.

Who your information is shared with

Your name, address and date of birth will appear on the medication package label and will be shared only as necessary with:

  • Practice staff involved in preparing and delivering medication
  • The staff at the village shop you have chosen, solely for the purpose of making the medication available for collection

Village shops:

  • Do not have access to your medical records
  • Do not retain records relating to your medication collection
  • Do not record details of the medication being supplied
  • Hold the medication only until it is collected or returned to the surgery

How your information is kept safe

We protect your information by:

  • Using sealed medication packaging
  • Sharing only the minimum necessary information
  • Limiting how long medication is held at the shop
  • Training staff in confidentiality and data protection
  • Having clear procedures for lost, damaged or uncollected medication

How long information is kept

  • Village shops do not retain records relating to medication collection.
  • Prescription and dispensing records are retained by the practice in accordance with NHS Records Management requirements.
  • Any uncollected medication will be returned to the surgery in line with practice procedures.

Your rights

You have the right to:

  • Ask how your information is used
  • Request access to your information
  • Request correction of inaccurate information
  • Withdraw from this arrangement at any time - Withdrawing will not affect your care; it will only change how you collect medication.
