Noticeboard

Due to a major incident in the village on Monday 1st June, we currently only have one working telephone line, and no internet or access to our clinical system within the surgery.

We are working hard to find alternative solutions, but in the meantime politely request that you only contact or visit the surgery if you have an urgent issue or prescription request.

Thank you for your understanding

CORONAVIRUS

Please see 'Latest News' in Quick Links or 'COVID 19' in the Further Information section on the right hand side of the screen.

Do It Online

The functionalities of our online system are ever increasing, you are able to order your repeat prescriptions, book appointments, view your Summary Record and complete questionnaires.

If you would like to arrange access to SystmOnline, please ring  the surgery and our Reception team will arrange this for you or you can download the Patient Online Application Form and Patient Information leaflet here.

If you wish to apply for access to someone else's records, you will need to complete the Proxy Access Application Form  here and the patient themselves will need to sign it, before a GP will determine if this is acceptable.

At Whiteparish Surgery, we also use email communication to invite our patients in to clinics where appropriate or to update them on any information from the practice. We do not use this method of communication for any confidential information.

If you would like to receive non-confidential information from us via email, simply send your Full Name, Date of Birth and Address to surgery.whiteparish@nhs.net from the email address you wish to be stored on your record.
