Chronic Disease Management

The practice takes a positive approach to the management of patients suffering from a wide group of chronic disease including:

  • chdAsthma
  • Chronic obstructive pulmonary disease (COPD)
  • Coronary Heart Disease
  • Diabetes
  • Hypertension

Patients falling within the above categories will be kept under review and if it is felt necessary will be invited to see a nurse practitioner, practice nurse or doctor to monitor their condition. It is helpful for patients to respond to these invitations and not ignore them. If you do not wish to be called periodically for a review you may be asked to sign a disclaimer to confirm that you are aware that you have been invited but have no wish to attend.

Services Offered

Blood Tests

Blood tests for patients registered with the practice are performed by a phlebotomist and healthcare assistant by appointment on most weekday mornings. Appointments are only available during the morning because the samples are collected at lunchtime to go to the hospital for processing.

Alternatively, you can go to the hospital to have your blood tests as long as you take your blood form with you. For more information, ask reception.

Contraception

We are able to provide a full range of contraception advice and methods. All doctors and the nurse practitioner are able to offer advice on all forms of contraception.

Antenatal Care

This is shared between your doctor and the hospital together with the community midwife, as well as an initial home visit by the health visitor to discuss your family's health needs.

dr_and_babyImmunisation

Immunisation is a way of protecting your child against serious disease. Once children have been immunised their bodies can fight those diseases if they come into contact with them. For more information about immunising babies and children visit the NHS immunisation website.

We offer immunisations for babies and pre-school children along with Child Health Surveillance on an appointment basis.

Influenza Vaccine

We offer flu vaccinations annually in the autum for our patients, especially those at particular risk e.g. those over 65 or suffering from chest problems, heart disease, asthma or diabetes. No appointment is necessary and clinics are advertised well in advance. No fee is charged.

Pneumonia Vaccine

We offer a single vaccination to all patients with coeliac disease, no spleen and other chronic conditions, and increasing age. The vaccine is often given at the same time as the flu vaccine. However, the majority of people only require a pneumonia vaccine once. If you think you should have one please seek advice from the nurse or doctor.

physioPhysiotherapy

We have a physiotherapist who works twice a week at the practice, and when the nurse practitioner or doctor considers it appropriate patients may be referred either to her or to the hospital.

Blood Pressure

We monitor patients with blood pressure problems regularly. Please ensure you attend for a blood pressure check as a minimum twice yearly.

Counselling

We have a visiting counsellor once a week at the practice and patients may be referred when the nurse practitioner or doctor considers it appropriate.

Cervical Smears

All women should have a cervical smear at least every five years (3-yearly for 24-49 years, 5-yearly for 50-65 years). This can be arranged with the practice nurse, nurse practitioner or doctor and applies to women who are sexually active or have been in the past, up to the age of 65. The practice will send a reminder when you smear is overdue. Should you not wish to attend a form is available to decline the invitation.

Private Medicals, Reports and Certificates

We offer a wide range of non-NHS services, including medicals for life insurance purposes, HGV, PSV etc. We also produce a wide range of reports and certificates as requested on an individual basis. Since these are not covered under the NHS we have to charge a fee. Details are available at reception.

Special Services and Clinics

Baby Clinics

These are run by the health visitor and details are available directly from her. Developmental checks and health promotion will be individually agreed with you. She will discuss the immunisation programme with you.

Immunisation

The practice patients have a high uptake of immunisations and all the doctors and members of the primary healthcare team are very keen to continue this. Immunisation can prevent several distressing illnesses and some life threatening conditions. If you have any doubts please discuss them with us so that we can help you to assess the possible risks.
