Electronic blood test request forms are now being generated by the Clinician for your blood test appointment. Paper blood test forms are still required if you have your blood sample taken at the hospital.

If you are requested to a have a fasting blood test this requires you to have nothing to eat or drink apart from water 12 hours before your appointment.
